The Stay Healthy and Active! Vaccination campaign created by the NC Division of Public Health's Immunization Branch presents a health care professional packet with evidence-based methods and materials to increase adult immunization rates.

The Branch encourages you to use the materials and strategies listed below to improve adult immunization rates. These patient education materials may be ordered at no cost by faxing the Materials Order Form to the NC Immunization Branch at 1-800-544-3058.

By implementing proven strategies and reminding patients to be immunized, more adults will receive low-cost, lifesaving vaccines and be able to "stay healthy and active"! With your help, we can improve the health of all North Carolinians!
